This study aimed to examine the association between clusters of physical activity (PA), diet, and television viewing (TV) with weight status among a representative sample of Brazilian students. Data from the National Health School-based Survey (PeNSE) 2015 were analyzed (n = 16,521; mean age 14.8, standard deviation 0.03 year). PA (minutes/week spent in leisure-time, and commuting to/from school), TV (hours/day), and weekly consumption of deep-fried empanadas, candies, sodas, ultra-processed foods, fast foods, green salads or vegetables, and fruits were self-reported on the validated PeNSE questionnaire. Latent class analysis defined behavior classes, and binary logistic regression assessed the association between clustering and weight status. Six classes' types with positive and negative behaviors were identified. Adolescents belonging to the "low TV time and high healthy diet" class had higher chances of being overweight (including obesity) compared to their peers in the "moderate PA and mixed diet" class. No associations were found in the other clusters. Mixed classes with healthy and unhealthy behaviors characterized adolescents' lifestyles and these profiles were related to weight status.

Resumo O estudo objetivou examinar a associação entre agrupamentos de atividade física (AF), dieta e tempo de televisão (TV) com o status do peso em uma amostra representativa de estudantes brasileiros. Foram analisados ​​dados da Pesquisa Nacional de Saúde do Escolar (PeNSE) 2015 (n = 16.521; idade média 14,8, desvio padrão de 0,03 ano). AF (minutos/semana no lazer e deslocamento para/da escola), assistir televisão (TV) (horas/dia), consumo semanal de doces, refrigerantes, alimentos ultraprocessados/fast-foods, saladas/vegetais e frutas foram coletadas por questionário validado. Análise de classes latentes definiu os perfis de comportamento e a regressão logística binária a associação entre agrupamento e status de peso. Foram identificadas seis classes com comportamentos positivos e negativos. Adolescentes pertencentes à classe "baixo tempo de TV e alta alimentação saudável" apresentaram maior probabilidade de ter sobrepeso/obesidade em comparação com seus pares na classe "AF moderada e dieta mista". Não foram encontradas associações nos outros perfis. Classes com comportamentos saudáveis ​​e não saudáveis ​​caracterizaram o estilo de vida dos adolescentes e estiveram relacionadas com o status de peso.

The juçara palm tree produces a small spherical and black-purple fruit similar to açaí. It is rich in phenolic compounds, especially anthocyanins. A clinical trial evaluated the absorption and excretion of the main bioactive compounds in urine and the antioxidant capacity in serum and erythrocytes of 10 healthy subjects after juçara juice intake. Blood samples were collected before (0.0 h) and 0.5 h, 1 h, 2 h, and 4 h after a single dose (400 mL) of juçara juice, while urine was collected at baseline and 0-3 and 3-6 h after juice intake. Seven phenolic acids and conjugated phenolic acids were identified in urine deriving from the degradation of anthocyanins: protocatechuic acid, vanillic acid, vanillic acid glucuronide, hippuric acid, hydroxybenzoic acid, hydroxyphenylacetic acid, and ferulic acid derivative. In addition, kaempferol glucuronide was also found in urine as a metabolite of the parent compound in juçara juice. Juçara juice caused a decrease in the total oxidant status of serum after 0.5 h in comparison to baseline values (p < 0.05) and increased the phenolic acid metabolites excretion. This study shows the relationship between the production of metabolites of juçara juice and the total antioxidant status in human serum, indicating evidence of its antioxidant capacity.

Longitudinal study, whose objective was to evaluate of the time trend in food consumption across the 2002-2015 period in schoolchildren aged 7 to 11 years, covered five food surveys in Florianopolis, southern Brazil. Methodological differences across the surveys (typical vs. previous day food consumption, pen-and-paper versus computer screen presentation) and some known risk factors, were adjusted for statistically. Offset by maximum food/beverage consumption per day allowed comparability of a varying number of food components in a food group over survey years. Significant reduction in some healthy (fruits and animal proteins) and unhealthy diet markers (sodas, sugary drinks, sweets) was observed over the 2013-2015 period compared with the 2002 and 2007 data. Computer screen presentation of the food questionnaire systematically reduced the reporting frequency compared to the pen-and-paper presentation. Converting frequency reports into rates and using contrast analysis allowed time trend estimation based on food questionnaires with different presentation modes, varying number of items within food group over survey years, and variations in probabilistic sampling procedures.

We analyzed the association between school attendance and the daily frequency of use of different forms of screens (TV, computer, cell phone and video game) among children and adolescents. Lon-gitudinal study with four repeated measures during the 2015 school year. Children and adolescents from a public school participated in the study (n = 463, 53.6% boys; 9.3 ± 1.3 years old). The daily frequency of screen use was the outcome analyzed, which was reported on an online questionnaire for the previous-day recall, illustrated with 32 icons of activities including 4 types of electronic de-vices. The participant answered "What did you do yesterday?" in the morning, afternoon, and night. School attendance was assessed by responding to the item "Did you go to school yesterday?". The analysis was conducted using generalized estimation equations, with adjustment by sex, age, BMI z-scores, and season of the year. The average percentage of absences during the follow-up was 28.7%. The use of screens was 13% higher among students who missed class. Playing a video game was 37% higher among students who missed class. The daily frequency of screen use was 26% higher among adolescents (10-12 years) compared to children (7-9 years), especially in the analysis performed indi-vidually for TV and cell phone. The daily frequency of screen use and watching TV among students who missed classes during the winter was, respectively, 24% (IRR = 1.24; 95%CI = 1.11 - 1.39) and 35% (IRR = 1.35; 95%CI = 1.10 - 1.66) higher compared to those who missed classes during the fall. In conclusion, the use of screens predominated among students who missed class, mainly in winter


Nós analisamos a associação longitudinal entre presença na escola e frequência diária de uso de diferentes tipos de telas (TV, celular, computador e videogame). O follow-up incluiu quatro medidas repetidas durante o ano letivo de 2015. Participaram do estudo crianças e adolescentes de escola pública (n = 463; 53,6% meninos; 9,3 ± 1,3 anos). O uso de telas foi o desfecho analisado, relatado em um questionário online para a recordação do dia anterior, ilustrado com 32 ícones de atividades, incluindo 4 tipos de dispositivos eletrônicos. O participante respondeu "O que você fez ontem?" pela manhã, tarde e noite. A frequência escolar foi avaliada por meio do item "Você veio à escola ontem?". A análise foi conduzida via equações de estimativa generalizada, com ajuste por sexo, idade, estação do ano e escore-z de IMC. O percentual médio de faltas no período foi de 28,7%. De maneira geral, o uso de telas foi 13% mais frequente entre estudantes que faltaram à aula, com destaque para vídeogame, cuja frequência diária foi 37% maior. Adolescentes (10-12 anos) exibiram frequência diária de telas 26% maior quando comparados com as crianças (7-9 anos), especialmente de TV (36%) e celular (32%). A frequência diária de uso de telas e de assistir TV entre os alunos que faltaram às aulas no inverno foi, respectivamente, 24% (IRR = 1,24; IC95% = 1,11 - 1,39) e 35% (IRR = 1,35; IC95% = 1,10 - 1,66) maior em relação aos que faltaram às aulas no outono. Concluímos que o uso de telas predominou entre estudantes que faltaram à aula, sobretudo no período do inverno

Resumo Estudo longitudinal cujo objetivo foi avaliar a tendência temporal do consumo alimentar no período 2002-2015 em escolares de 7 a 11 anos, abrangendo cinco inquéritos alimentares em Florianópolis, Sul do Brasil. Foi ajustado para diferenças metodológicas (consumo alimentar típico vs. dia anterior, caneta e papel vs. apresentação no computador) e alguns fatores de risco. A compensação pelo consumo máximo de alimentos/bebidas por dia permitiu a comparabilidade de um número variável de componentes de alimentos em um grupo de alimentos ao longo dos anos da pesquisa. Foi observada redução em alguns marcadores de dieta saudável (frutas e proteínas animais) e não saudáveis (refrigerantes, bebidas açucaradas, doces) no período de 2013-2015, em comparação com os dados de 2002 e 2007. A apresentação do questionário alimentar na tela do computador reduziu sistematicamente a frequência de relatórios em comparação com a apresentação em papel e caneta. A conversão de relatórios de frequência em taxas e o uso de análise de contraste permitiram a estimativa de tendência de tempo com base em questionários de alimentos com diferentes modos de apresentação, número variável de itens dentro do grupo de alimentos ao longo dos anos da pesquisa e variações nos procedimentos de amostragem probabilística.

OBJECTIVE: Assess two validity evidences of the diet quality scale (ESQUADA) for the selection of items with better discrimination of the Brazilians' diet quality and propose a description in score levels. METHODS: Brazilian adolescents and adults residing in the country (n = 2,059) answered an online questionnaire with 52 items, shared on social networks and email lists between March and April 2018. Statistical tests were applied to analyze the validity and reliability of the instrument's evidence. Factor analysis was applied to study the dimensionality of the questionnaire items. Item response theory was applied to identify the discrimination and location of items on the continuum, construct the scale and assess the differential item functioning in terms of sex and age. RESULTS: Among the 52 items of the questionnaire, 25 had greater measurement accuracy, with adequate adjustment and reliability. The item on the habit of eating ultra-processed foods at home showed the best discrimination of diet quality. No item showed differential functioning regarding sex and age. In the construction of the ESQUADA, five diet quality levels were identified: very poor, poor, good, very good and excellent. It was observed that while breakfast cereals and/or cereal bars are more frequently consumed by individuals with "very poor" diet quality; nuts and/or walnuts are most often consumed by those individuals with "excellent" diet quality. CONCLUSION: The ESQUADA consists of 25 precise items with no differential functioning to assess the quality of Brazilians' diet. The construction of the ESQUADA made it possible to recognize food consumption and dietary practices characteristic of each level of diet quality.

Este estudo analisou a variação sazonal das atividades físicas e dos comportamentos sedentários de uma amostra por conveniência de crianças e adolescentes, de uma escola pública no semiárido baiano (n = 462; 52,9% de meninos; 7-12 anos). Trata-se de um estudo longitudinal de um ano de seguimento. Atividades físicas e comportamentos sedentários foram relatados no questionário Consumo Alimentar e Atividade Física de Escolares (Web-CAAFE). As medidas repetidas foram realizadas no outono, inverno e primavera. Os desfechos avaliados foram: quantidade de atividades físicas diária (AFD) e de comportamentos sedentários (CS), frequências de atividades físicas leves (AFL), moderadas (AFM) e vigorosas (AFV ). Análises foram conduzidas via modelagem de equações de estimativa generalizada (GEE) (distribuição Poisson), com ajuste por temperatura ambiente e precipitação pluviométrica. Não houve variação sazonal estatisticamente significativa na AFD. No inverno, houve mais relatos de AFL (Coef² = 1,37; IC95%: 1,14-1,66) e de CS (Coef² = 1,16; IC95%: 1,05-1,29), enquanto AFV foram menos frequentes (Coef² = 0,83; IC95%: 0,71-0,97). AFV foram menos relatadas no outono (Coef² = 0,82; IC95%: 0,69-0,98). Sexo modificou o efeito das estações nas AFV, que entre meninos, ocorreram mais no outono (Coef² = 1,72 IC95%=1,16-2,56) e no inverno (Coef² = 1,84; IC95%: 1,26-2,69). Idade modificou efeito das estações sobre os CS. Durante o seguimento, houve mais CS na faixa etária 10-12 anos, porém, com diferença estatística significativa no outono e no início da primavera. O inverno favoreceu as AFL e os CS, mas restringiu as AFV, sobretudo entre meninas. Estudantes de 10-12 anos apresentaram mais CS, especialmente no outono e início da primavera


This one-year follow-up study analyzed the seasonal variation of physical activities and sedentary behaviors among children and adolescents, from public schools in the semiarid region of Bahia state (convenience sample, n = 462; 52.9% boys; 7-12 years). Physical activities and sedentary behaviors were reported in the Food Intake and Physical Activity of School Children Questionnaire ( WEB-CAAFE). Seasonal measure-ments were performed in fall, winter and spring. The outcomes were: daily amount of physical activities (DPA) and sedentary behaviors (SB), frequencies of light (LPA), moderate (MPA) and vigorous ( VPA) physical activities. The analyses were conducted via modeling of generalized estimation equations (GEE), with adjustment by temperature and rainfall. There was no statistically significant seasonal variation in DPA. During winter, there were more reports of LPA (Coef² = 1.37; 95%CI: 1.14-1.66) and SB (Coef² = 1.16; 95%CI: 1.05-1.29), while VPA was less frequent (Coef² = 0.83; 95%CI: 0.71-0.97). VPA occurred less in the fall (Coef² = 0.82; 95%CI: 0.69-0.98). Sex modified the effect of seasons on VPA: among boys, VPA occurred more frequently in the fall (Coef² = 1.72; 95%CI: 1.16-2.56) and winter (Coef² = 1.84; 95%CI: 1.26-2.69). Age modified the effect of the seasons on the SB: there were more SB in the 10-12 years age group, however, with a statistically significant difference only in the fall and early spring. LPA and SB increased during winter, but VPA decreased, especially among girls. Students aged 10-12 years had more SB in the fall and early spring

OBJECTIVE: The aim of the current study was to identify and describe the meal and snack patterns (breakfast, mid-morning snack, lunch, mid-afternoon snack, dinner and evening snack) of public schoolchildren. DESIGN: Cross-sectional study. Information on the previous day's food intake was obtained through the Web-CAAFE (Food Intake and Physical Activity of Schoolchildren), an interactive questionnaire, which divides daily food consumption into three meals (breakfast, lunch and dinner) and three snacks (mid-morning, mid-afternoon and evening). Each meal contains thirty-one food items and the schoolchildren clicked on the food items consumed in each meal. Factor analysis was used to identify meal and snack patterns. The descriptions of the dietary patterns (DP) were based on food items with factor loads ≥ 0·30 that were considered representative of each DP. SETTING: Schoolchildren, Florianopolis, Brazil. PARTICIPANTS: Children (n 1074) aged 7-13 years. RESULTS: Lunch was the most consumed meal (96·0 %), followed by dinner (86·4 %), breakfast (85·3 %) and mid-afternoon snack (81·7 %). Four DP were identified for breakfast, mid-morning snack, lunch, dinner and evening snack, and three for mid-afternoon snack. Breakfast, lunch and dinner patterns included traditional Brazilian foods. DP consisting of fast foods and sugary beverages were also observed, mainly for the evening snack. CONCLUSIONS: The results of the current study provide important information regarding the meal and snack patterns of schoolchildren to guide the development of nutrition interventions in public health.

Background: Multiple modifiable lifestyle factors are well-known contributors to many health problems. Objectives: This study aims to determine the association between latent class analysis (LCA) of modifiable lifestyle risk factors with being overweight and/or obese for children and/or adolescents. Methods: Articles were selected from six databases, without limitation regarding language or date. The review included studies that identify latent classes of modifiable lifestyle risk factors [e.g., physical activity (PA), diet, sedentary behavior (SB), and/or unhealthy behavior] by LCA to determine the association between latent classes with being overweight and/or obese. The methodology of the selected studies was evaluated using the JBI Critical Appraisal Checklist for Analytical Cross-Sectional Studies. Owing to the heterogeneity between latent classes of modifiable lifestyle risk factors with obesity and/or being overweight, the results are described narratively. Results: Using a selection process in two phases, nine articles were included. All of the included studies were of high methodological quality. The studies were conducted in six different countries: the USA, Brazil, Canada, Portugal, Italy, and Australia. Sample sizes ranged from 166 to 18.587 children and adolescents, and in terms of age (range 5-19 years). Across study clusters characterized by low consumption of fruit and vegetables, and high consumption of fatty foods, sugar snack foods, sweets, chips and fries, low PA (<1 hour each day), and high SB (screen time and TV >2 hours/day), sleep time (<10 hours/day) were positively associated with being overweight and/or obese. Conclusion: Overall there is good evidence to support that the modifiable lifestyle risk factors clustered together by LCA should be novel targets for the treatment of obesity and its associated comorbidities.

RESUMO OBJETIVO Avaliar duas evidências de validade da escala de qualidade da dieta (ESQUADA) para seleção dos itens com melhor discriminação da qualidade da dieta dos brasileiros e propor uma descrição em níveis de escore. MÉTODOS Adolescentes e adultos brasileiros e residentes no país (n = 2.059), responderam a um questionário on-line com 52 itens, compartilhado em redes sociais e listas de correio eletrônico entre março e abril de 2018. Foram aplicados testes estatísticos para análise de evidências de validade e confiabilidade do instrumento. A análise fatorial foi aplicada para estudo da dimensionalidade dos itens do questionário. A teoria de resposta ao item foi aplicada para identificar a discriminação e localização dos itens no continuum , construir a escala e avaliar o comportamento diferencial dos itens quanto ao sexo e idade. RESULTADOS Dentre os 52 itens do questionário, 25 apresentaram maior precisão de medida, com ajuste e confiabilidade adequados. O item sobre o costume de comer alimentos ultraprocessados em casa apresentou a melhor discriminação da qualidade da dieta. Nenhum item apresentou comportamento diferencial quanto a sexo e idade. Na construção da ESQUADA foram identificados cinco níveis de qualidade da dieta: "muito ruim", "ruim", "boa", "muito boa" e "excelente". Observou-se que enquanto cereais matinais e/ou barrinhas de cereais são consumidos com maior frequência por indivíduos com qualidade da dieta "muito ruim"; castanhas e/ou nozes são consumidos mais frequentemente por aqueles indivíduos com qualidade da dieta "excelente". CONCLUSÕES A ESQUADA é composta por 25 itens precisos e sem comportamento diferencial para avaliar a qualidade da dieta dos brasileiros. A construção da ESQUADA possibilitou reconhecer consumo e práticas alimentares característicos de cada nível de qualidade da dieta.

OBJECTIVES: To assess the association between sleep period time and dietary patterns (DPs) in Brazilian schoolchildren aged 7-13 years. METHODS: Sleep data of 1019 children were obtained from a questionnaire completed by parents/guardians. Food consumption, physical activity, and screen-based activity were assessed through the validated Web-CAAFE questionnaire. Latent profile analysis was used to identify dietary patterns. Associations between sleep period time and dietary patterns were analyzed by multinomial logistic regression, and the average marginal effect of sleep period time on the probability of adherence to dietary patterns was determined. RESULTS: Overall, 11.4% of children had inadequate sleep period time (<9 h per day). Three mutually exclusive dietary patterns were identified: "Monotonous" characterized by high consumption of pasta, instant pasta, sweets, and hamburgers/hot dog/pizza/salty snacks; "Traditional" characterized by high consumption of rice, vegetables, green leaves, beans, manioc flour, maize/potatoes, and meats; and "Dairy, bread, green leaves, maize/potatoes and sausage". The likelihood of adherence to the "Dairy, bread, green leaves, maize/potatoes and sausages" DP decreased by 0.026 as sleep period time increased by 1 h per day. For a sleep period time of more than 10 h per day, the average marginal effect of sleep period time on the likelihood of adherence to the this dietary pattern was significant and negative. CONCLUSIONS: Sleep period time was inversely associated with adherence to the "Dairy, bread, green leaves, maize/potatoes and sausages" DP; children who slept more than 10 h per day were less likely to present this pattern.

INTRODUCTION: The Food Intake and Physical Activity of School Children (Web-CAAFE) is an online questionnaire developed to monitor the diet and physical activity of Brazilian schoolchildren. The study aimed to evaluate the effect of the day of the week and the number of applications on Web-CAAFE reproducibility. METHODS: Schoolchildren in grades 2 to 5 of two municipal schools in Florianópolis (n = 197) filled out the Web-CAAFE twice in the same day, of which 113 filled out the instrument in three non-consecutive days. The analysis was performed using the intraclass correlation (ICC) and logistic regression was used to evaluate the factors associated with poor reproducibility. RESULTS: The reproducibility of the questionnaire was significantly lower for children who filled out their consumption mostly on weekends in relation to weekdays (OR = 2.93; p = 0.045), and was significantly better in girls in relation to boys (OR = 0.29; p< 0.001). Higher CCI was observed on the second and third day of Web-CAAFE application in relation to the first day. These results are relevant for planning future studies that use dietary assessment instruments in children, especially on evaluation of their validity and reproducibility. CONCLUSIONS: The results suggest that at least two days provides acceptable reproducibility results of the Web-CAAFE and that the weekend recall may reduce the reproducibility.

Background: Childhood overweight and obesity are recognized as predictors of the risk of obesity in adulthood. The aim of this systematic literature review was to determine the association between dietary pattern and obesity risk among children. Methods: Articles were selected from databases (Cochrane Library, Lilacs, Eric, Livivo, and PubMed/Medline), without limitations regarding language or date. Database-specific search terms included the key words "obesity," "diet," "dietary pattern," "childhood," "children," "adolescents," and relevant synonyms. The review included studies that reported the assessment of the dietary pattern in childhood and that correlated eating patterns with the obesity risk through cluster analysis (CA) and/or factor analysis and/or principal component analysis and presented odds ratios (ORs). The methodology of the selected studies was evaluated using the JBI Critical Appraisal Checklist for Analytical Cross-Sectional Studies. Owing to the heterogeneity among the assessments of the association of dietary patterns and obesity, the results are described narratively. Results: Using a selection process in two phases, 16 articles were included. Fifteen studies used a cross-sectional design, and one case-control study. The included studies showed variation in sample size (range = 232 to 10,187 children/adolescents) and age (range = 1-19 years old). The studies reported between two and five dietary patterns each. The OR for the relationship between the dietary pattern and the risk of childhood obesity ranged from OR = 1.02; confidence interval (95% CI) (0.91-1.15) to OR = 3.55; 95% CI (1.80-7.03). In this study, the dietary patterns identified by the studies were given different names. The food intake patterns identified could, in most factor or CA studies, be categorized as (1) potentially obesogenic foods that increased risk of becoming overweight (including fatty cheeses, sugary drinks, processed foods, fast food, candies, snacks, cakes, animal products, whole milk, and refined grains) or (2) food classified as healthy with the weakest association with the risk of becoming overweight or obese (including low levels of sugar and fat and high levels of fruits, vegetables, whole grains, fish, nuts, legumes, and yogurt). Conclusion: Overall, the results indicated from most studies that a diet with a lower percentage of obesogenic foods should be effective in reducing the risk of developing obesity.

ABSTRACT Objective To identify trends in food consumption among schoolchildren (2nd-5th grades) from public schools in the city of Florianópolis, Brazil, in a period of three years. Methods Three cross-sectional surveys were carried out in 2013 (n=1,942), 2014 (n=1,989) and 2015 (n=2,418). Dietary intake data were obtained using the Web-Based Food Intake and Physical Activity of Schoolchildren questionnaire. Food items were aggregated to eight food groups. Kruskal-Wallis heterogeneity and trend tests were used to analyze the differences and trends among the mean intake frequency of food groups. Results There were trends to decrease the mean intake frequency of sweets in the total sample (2013: 0.72±0.91; 2014: 0.68±0.87; 2015: 0.67±0.89, p=0.03) which was determined by children between 7-9 years old (2013: 0.69±0.88; 2014: 0.64±0.85; 2015: 0.62±0.87, p=0.02), and boys (2013: 0.75±0.90; 2014: 0.70±0.86; 2015: 0.68±0.88, p=0.03). Younger children also tended to increase the mean intake frequency of fruits and vegetables (2013: 1.03±1.35; 2014: 1.16±1.45; 2015: 1.17±1.41, p=0.03) and those aged ten-12 years decreased their intake of dairy products (2013: 1.32±1.25; 2014: 1.23±1.18; 2015: 1.20±1.20, p=0.05). Conclusion The results suggest positive trends for younger children, with an increased consumption of fruits and vegetables in both sexes and decreased consumption of sweets for boys. Older children reduced their consumption of dairy products over the three-year period of this study.


RESUMO Objetivo Este estudo buscou identificar tendências no consumo alimentar de escolares do 2º ao 5º ano de escolas públicas do município de Florianópolis durante três anos. Métodos Três pesquisas transversais foram conduzidas em 2013 (n=1.942), 2014 (n=1.989) e 2015 (n=2.418). Dados de consumo alimentar foram obtidos com a aplicação do questionário de avaliação de consumo alimentar e atividade física. Os itens alimentares foram classificados em oito grupos. Utilizaram-se os testes Kruskal-Wallis de heterogeneidade e de tendência para analisar as diferenças e tendências entre as médias de frequência de consumo dos grupos alimentares. Resultados Verificou-se uma tendência de redução da média de frequência de consumo de doces para a amostra total (2013: 0,72±0,91; 2014: 0,68±0,87; 2015: 0,67±0,89, p=0,03), que foi determinada pelos escolares de 7 a 9 anos (2013: 0,69±0,88; 2014: 0,64±0,85; 2015: 0,62±0,87, p=0,02) e do sexo masculino (2013: 0,75±0,90; 2014: 0,70±0,86; 2015: 0,68±0,88, p=0,03). Escolares mais novos também tenderam a aumentar o consumo de frutas, verduras e legumes (2013: 1,03±1,35; 2014: 1,16±1,45; 2015: 1,17±1,41, p=0,03), e os de 10 a 12 anos diminuíram laticínios (2013: 1,32±1,25; 2014: 1,23±1,18; 2015: 1,20±1,20, p=0,05). Conclusão Os resultados sugerem tendências positivas para escolares mais novos, com o aumento do consumo de frutas, verduras e legumes em ambos os sexos e redução de doces para os meninos. Escolares mais velhos reduziram laticínios nos três anos de seguimento.

ABSTRACT Objective To assess the outcome of an educational nutritional intervention in the quality of diet of women with breast cancer in adjuvant treatment. Methods Women with breast cancer and admitted for surgical treatment were divided in an intervention group (n=18) and a comparison group (n=78), and participated in a nonrandomized clinical trial. Participants were assessed before and after the treatment and/or intervention. A food frequency questionnaire was applied and the quality of diet was calculated using the Brazilian Healthy Eating Index Revised. The educational nutritional intervention lasted 12 months and was carried out through phone calls twice a month, personal meetings, and monthly handouts. It oriented an intake of at least 400g of vegetables and fruits per day and at most 500g of red/processed meat per week. Results We observed a significant improvement in the diet quality of the intervention group (+5.7). Nutritional education in the intervention group reached its objectives, with a rise in the intake of vegetables and fruits reflecting a significant increase in the score of total fruits (+1.9) and whole fruits (+1.1), and a decrease in the score of meat, eggs, and legumes (-3.3) by means of a reduction in the intake of red/processed meat (p<0.05). Also, a rise in the punctuation of calories from solid fats, alcohol, and added sugar was observed (+5.0, p<0.05) due to lower intake of these components. The comparison group presented no significant differences. Conclusion The nutritional intervention contributed to improving the quality of the diet during breast cancer treatment and possibly altered these women's prognoses.


RESUMO Objetivo O estudo objetivou analisar o impacto de uma intervenção nutricional educativa na qualidade da dieta de mulheres com câncer de mama em tratamento adjuvante. Métodos Mulheres com câncer de mama admitidas para tratamento cirúrgico, divididas em grupo intervenção (n=18) e grupo comparação (n=78), participaram de um ensaio clínico não randomizado. As participantes foram avaliadas antes e após o tratamento e/ou intervenção. Um questionário de frequência alimentar foi aplicado, e a qualidade da dieta foi calculada pelo Índice de Qualidade da Dieta Revisado. A intervenção nutricional educativa ocorreu por meio de ligações quinzenais, encontros presenciais e materiais informativos, durante 12 meses, com orientações visando à ingestão de pelo menos 400g/dia de vegetais e frutas, e não mais que 500g/semana de carne vermelha/processada. Resultados O grupo de intervenção melhorou significativamente a qualidade da dieta (+5,7). Além disso, houve aumento da ingestão de frutas e vegetais, representado por elevação significativa na pontuação de frutas totais (+1,9) e frutas integrais (+1,1) e redução considerável na pontuação de carnes, ovos e leguminosas (-3,3), a qual se deu pela diminuição do consumo de alimentos embutidos e carne bovina (p<0,05). Ainda, observou-se aumento na pontuação do componente que avalia as quilocalorias provenientes das gorduras sólidas, bebidas alcoólicas e açúcar de adição (+5,0, p<0,05) devido ao menor consumo desses alimentos. O grupo comparação não apresentou diferenças significativas. Conclusão A intervenção nutricional contribuiu para melhorar a qualidade da dieta durante o tratamento para câncer de mama e, possivelmente, o prognóstico dessas mulheres.

RESUMO: Introdução: O questionário de Consumo Alimentar e Atividade Física de Escolares (Web-CAAFE) foi desenvolvido para o monitoramento da dieta e atividade física de escolares brasileiros. Este estudo buscou avaliar o efeito do dia da semana e do número de aplicações na reprodutibilidade do Web-CAAFE. Metodologia: Escolares do 2º ao 5º ano (n = 197) de Florianópolis preencheram o Web-CAAFE por duas vezes no mesmo dia, dos quais 113 o fizeram em três dias não consecutivos. Avaliou-se a reprodutibilidade por meio da correlação intraclasse (CCI) e da regressão logística, para avaliação dos fatores associados à pior reprodutibilidade. Resultados: Observaram-se piores resultados de reprodutibilidade para o preenchimento do consumo nos finais de semana em relação àqueles que responderam em dias da semana (odds ratio - OR = 2,93; p = 0,045) e melhores resultados em meninas em relação aos meninos (OR = 0,29; p < 0,001). O preenchimento do consumo realizado nos segundo e terceiro dias apresentou melhor reprodutibilidade que o realizado no primeiro. Esses resultados são relevantes para o planejamento de futuros estudos que utilizem instrumentos de consumo alimentar em crianças, em especial sobre avaliação de sua validade e reprodutibilidade. Conclusões: Os resultados sugerem que pelo menos dois dias de aplicação do Web-CAAFE fornecem resultados aceitáveis de reprodutibilidade, e que o preenchimento referente ao fim de semana pode reduzi-la.

The aim of this study was to determine the contribution of individual and school characteristics to the variability in body mass index (BMI) z-scores of 7 to 10 years old children. Anthropometric and sociodemographic data from two cross-sectional studies conducted with schoolchildren from the 2nd to the 5th grades of elementary schools were analyses (n = 2,936 in 2002, and n = 1,232 in 2007). Multilevel modeling was used to estimate variations in BMI at child and school levels. The contribution of the school context to the overall variability of BMI z-score was small but significant in 2002 (3.3%-4.4%) and in 2007 (2.4%-5.3%), showing that schoolchildren from private schools had a higher BMI compared to those from public schools. The monthly family income showed, in general, a negative association with BMI z-score in 2002 and a positive association in 2007, for both sexes. The consumption of sweets showed a negative effect in the BMIs of children. In both surveys, overweight/obese mothers and excessive birth weight were positively associated with BMI z-score. Mother's weight status had a higher influence on the overall variability of BMI in both surveys. In conclusion, school and child characteristics contributed to the variance in children's weight status. The results imply that overweight/obesity childhood prevention programs should focus on strategies of family engagement to be more effective.

O estudo teve como objetivo medir a contribuição das características individuais do aluno e da escola à variabilidade dos escores-z do índice de massa corporal (IMC) em crianças brasileiras entre 7 e 10 anos de idade. Foram analisados dados antropométricos e sociodemográficos de dois estudos trnasversais conduzidos com escolares da segunda à quinta série de Ensino Fundamental (n = 2.936 em 2002 e n = 1.232 em 2007). A modelagem multinível foi utilizada para estimar as variações de IMC em nível individual e de escola. A contribuição do contexto escolar à variabilidade global do escore-z do IMC foi pequena, porém significativa, em 2002 (3,3%-4,4%) e em 2007 (2,4%-5,3%), mostrando que alunos de escolas particulares tinham IMC mais alto, comparado ao dos alunos de escolas públicas. A renda familiar mensal mostrou uma associação negativa com o escore-z do IMC em 2002 e uma associação positiva em 2007, para ambos sexos. O consumo de doces mostrou efeito negativo sobre o IMC das crianças. Em ambos estudos, filhos de mães com sobrepeso/obesidade e alunos com história de excesso de peso ao nascer mostraram associação positiva com o escore-z do IMC. Em ambos estudos, o estado nutricional materno mostrou influência maior sobre a variabilidade global do IMC. Em conclusão, as características da escola e do aluno contribuíram para a variância no IMC da criança. Os resultados sugerem que, para serem mais eficazes, os programas de prevenção de sobrepeso/obesidade na infância devem focar em estratégias com participação da família.


El objetivo de este estudio fue determinar la contribución de las características individuales y escolares en la variabilidad del índice de masa corporal (IMC) z-scores en niños de entre 7 a 10 años de edad. Se analizaron datos antropométricos y sociodemográficos de dos estudios transversales conducidos con escolares de la segunda a la quinta serie de la enseñanza fundamental (n = 2.936 en 2002 y n = 1.232 en 2007). Se utilizaron modelos multinivel para estimar variaciones en el IMC en los niveles infantiles y escolares. La contribución del contexto escolar para la variabilidad general de IMC z-score fue pequeña, pero significativa en 2002 (3,3%-4,4%) y en 2007 (2,4%-5,3%), indicando que los escolares de escuelas privadas tenían un IMC superior, comparados con los de las escuelas públicas. Los ingresos familiares mensuales mostraron, en general, una asociación negativa con el IMC z-score en 2002 y una asociación positiva en 2007, para ambos sexos. El consumo de dulces mostró un efecto negativo en el IMC de los niños. En ambas encuestas, las madres con sobrepeso/obesas y el peso excesivo al nacer se asociaron positivamente con el IMC z-score. El estatus del peso de la madre tuvo una influencia más alta en la variabilidad general del IMC en ambas encuestas. En conclusión, las características escolares y del niño contribuyeron a la variancia en el estatus de peso de los niños. Los resultados implican que los programas infantiles de prevención de sobrepeso/obesidad deberían enfocarse en estrategias de adhesión familiar para ser más efectivos.

INTRODUCTION: The quality control of data entry in computerized questionnaires is an important step in the validation of new instruments. The study assessed the consistency of recorded weight and height on the Food Intake and Physical Activity of School Children (Web-CAAFE) between repeated measures and against directly measured data. METHODS: Students from the 2nd to the 5th grade (n = 390) had their weight and height directly measured and then filled out the Web-CAAFE. A subsample (n = 92) filled out the Web-CAAFE twice, three hours apart. The analysis included hierarchical linear regression, mixed linear regression model, to evaluate the bias, and intraclass correlation coefficient (ICC), to assess consistency. Univariate linear regression assessed the effect of gender, reading/writing performance, and computer/internet use and possession on residuals of fixed and random effects. RESULTS: The Web-CAAFE showed high values of ICC between repeated measures (body weight = 0.996, height = 0.937, body mass index - BMI = 0.972), and regarding the checked measures (body weight = 0.962, height = 0.882, BMI = 0.828). The difference between means of body weight, height, and BMI directly measured and recorded was 208 g, -2 mm, and 0.238 kg/m², respectively, indicating slight BMI underestimation due to underestimation of weight and overestimation of height. This trend was related to body weight and age. CONCLUSION: Height and weight data entered in the Web-CAAFE by children were highly correlated with direct measurements and with the repeated entry. The bias found was similar to validation studies of self-reported weight and height in comparison to direct measurements.


INTRODUÇÃO: O controle da qualidade da entrada de dados em questionários informatizados é etapa importante na validação de novos instrumentos. Este estudo avaliou a consistência de registros de peso e altura no Questionário Consumo Alimentar e Atividade Física de Escolares (Web-CAAFE) entre aplicações repetidas e contra dados aferidos. MÉTODOS: Escolares do 2º ao 5º ano (n = 390) tiveram peso e altura aferidos e, em seguida, preencheram o Web-CAAFE. Uma subamostra (n = 92) completou o instrumento 2 vezes com 3 horas de intervalo. A análise incluiu regressão linear hierárquica, modelo misto de regressão linear, para avaliar vieses, e coeficiente de correlação intraclasse (CCI), para verificar a consistência dos dados. A regressão linear univariada avaliou o impacto de: sexo; desempenho na leitura/escrita; e posse e uso de computador/internet nos resíduos dos efeitos fixos e randômicos. RESULTADOS: O Web-CAAFE exibiu valores altos de CCI entre aplicações repetidas (peso = 0,996, altura = 0,937, índice de massa corporal - IMC = 0,972) e com relação às medidas aferidas (peso = 0,962, altura = 0,882, IMC = 0,828). A diferença entre as médias de peso, altura e IMC aferidos e digitados foi de 208 g, -2 mm e 0,238 kg/m², respectivamente, indicando leve subestimação do IMC em razão da subestimação do peso e da superestimação da altura. Essa tendência relacionou-se ao peso corporal e à idade. CONCLUSÃO: Peso e altura digitados no Web-CAAFE foram fortemente correlacionados com medidas diretas e digitação repetida.
